Once authorized by your supervisor, we provide immediate treatment should you be injured on the job. Our staff is experienced in dealing with the paperwork needed for on-the-job injuries, and we are able to connect you with area specialists or rehabilitative services as needed.

The following information should be brought with you or faxed to us at the time of your visit:

  • Verification from your company that you have incurred an injury while working and that your company is responsible for the bill.

At the end of your first visit, you will receive several forms to return to your supervisor, including:

  • a note stating your ability to do your job or modified duty

  • a green Workers' Compensation Request for Claim Number form, which should be filled out by your supervisor and returned to us by the time of your next visit. This form will have your claim number from your company's insurance carrier, and we will need this claim number in order to treat you after the first visit.


After your visit your company is responsible for:

  • Completing a First Report of Injury or Illness form and sending it to the Workers' Compensation Commission in Baltimore.

  • Calling the company's insurance carrier and reporting the incident, thereby obtaining a claim number for you.

  • Filling out our green Workers' Compensation Request for Claim Number form and returning it to Secure Medical Care by fax, mail, or asking you to bring it with you to your next visit.
