What you should know.

Patient Services

Put your coverage to work for you.

We’re a participating provider with many major health insurance plans, and a participating Medicare provider. It’s important to us that it is as convenient to pay for your services as it is to receive them.

We expect payment at the time of service. And we accept cash, personal check, debit card, or major credit card.

Is your insurance plan listed?
If we participate in your plan, you’ll only be responsible for your co-pay, and we’ll submit your claim for you. If your plan is not on our list, you’ll need to pay for your visit in full at the time of service. We’ll give you the forms you need to submit a claim to your insurer.

Many health plans require that you have a referral from your primary care physician before seeking treatment at an urgent care center, emergency department, or specialist. Please call your insurance company to verify your benefits and insure that you meet any pre-authorization requirements.

Fees - for services not covered by health insurance at our facility.

If you do not have health insurance, or if we do not participate with your health insurance plan, you will be asked to pay at the time of service. We accept cash, major credit cards, and checks.

Basic office visit – starts at $95, but this charge may increase based on the complexity of your visit. X-rays, blood tests, and other services are additional.

Immigration physical exams – $135
This includes the skin test for tuberculosis, as well as the blood tests for HIV/AIDS and syphilis. An x-ray or immunization is an additional charge (see Services).

Travel Medicine – $55
This includes consultation with a physician, printed information about your destination, and any prescriptions needed for malaria prophylaxis or travelers' diarrhea. Selected anti-malarial medications are available from our pharmacy at an additional cost. There will be an additional charge for any shots you may need.

Physical exams for sports, day care, school, camp, or other employment – $70 to $95

Don’t forget to bring your insurance card and driver’s license with you. We’ll need them to verify your insurance coverage. If you don't have your insurance card and photo ID with you at the time of your visit, we will ask you to pay for your visit in full and submit your own insurance claim.
