Things to Remember

 

  • If you wear eyeglasses, please bring them with you.
  • You will need to bring a note from your doctor if you take medication for a chronic disease.
  • You may be asked to submit to a urine drug screen at the time of your exam.
  • Your paperwork and license will be returned to you in hand or mailed to your employer, as appropriate.
